Birth Certificates

Chelan-Douglas Health District’s Vital Records office can provide official copies of birth certificates for anyone born in Washington State from 1907 to the present.

Qualified applicants for birth certificates are:

  • Subject of the record (a person who is buying their own birth certificate)
  • Spouse or domestic partner
  • Parent, step-parent, or legal guardian
  • Child or stepchild
  • Sibling
  • Grandparent or great grandparent
  • Grandchild
  • Legal representative
  • Authorized representative
  • Government agency or the courts if the birth certificate is used to conduct official duties

Information required for a birth certificate records request:

  • Name of child
  • Child’s birth date
  • City of Birth
  • Mother’s maiden name
  • Father’s full name

Note: Refunds are not available if a customer does not provide additional information within the 30-day window or if a record cannot be found.

Death Certificates

Chelan-Douglas Health District’s Vital Records office can provide certificates for deaths that occurred in 2015 to recent deaths only. The Washington State Department of Health’s Center for Health Statistics issues certificates for all deaths occurring in Washington from 1907 to present.

Qualified applicant for long form death certificates are:

  • Spouse or domestic partner of the decedent
  • Parent, step-parent, or legal guardian immediately prior to death
  • Child or stepchild
  • Sibling
  • Grandparent or great grandparent
  • Grandchild
  • Legal representative
  • Authorized representative
  • Next of kin, only if no one else from this list is living
  • Funeral director or funeral establishment that is named on the record (up to 12 months from the date of death)
  • Government agency or courts if the death certificate will be used for official duties

Qualified applicants for short form death certificates are the same as the long form death certificates, plus these additional qualified applicants:

  • A title insurer or title insurance agent handling a transaction involving real property
  • A person that demonstrates the certificate is necessary for a determination related to the death or protection of a personal or property right related to the death

Information required for a death certificate records request:

  • First, Middle, and last name of the deceased
  • Date of death (or a 10-year period to search)
  • City of death
  • Mother’s maiden name
  • Father’s full name

Note: If you choose to receive your certificate by mail, please allow between 1-3 weeks.

How Do I Order A Birth Or Death Certificate?

In Person

Visit the Chelan-Douglas Health District, where you can fill out certificate application form, pay the required fee (cash, check, credit and debit cards are accepted). Your order will be ready for pick up the next business day by 1 pm or you can choose the option to receive by mail. You may pay the expedite fee for same day service, if needed.

We are located at 200 Valley Mall Parkway in East Wenatchee across the street from Arby’s.

Business hours are 9 a.m. – 5 p.m. Monday through Thursday. Our office is closed on Friday. Please arrive no later than 4 p.m. to receive expedited same-day service.

By Mail

Complete a Birth Certificate Order Form  or Death Certificate Order Form  with a check or money order payable to “CDHD” and send to:

Note: The regular processing time is two business days plus mail time each way.
