Notify us about a foodborne illness or a food safety concern
Washington State Department of Health has launched a Foodborne Illness Notification System (FINS). You can use FINS to let us know about confirmed or suspected food poisoning or food safety concerns about a restaurant or place that serves food in Washington State.
